Submitting your self assessment
We'll be using a separate system for collating and verifying the self assessment evidence. We'll provide details of the system, timelines and how to upload your evidence to shortlisted applicants after longlisting has been completed.
We expect you to upload documentation relating to your score only for each question. You'll be advised of the dates to upload your evidence. Your documents will then be verified against your self assessment score by a panel of clinicians. After verification has been completed, applicants will be provided with feedback detailing any amended scores.
The verification panel will only be able to assess the evidence you provide. They will not contact you to request additional evidence.
This means you should provide all evidence in a clear format so it's easy to identify and assess.

Self assessment appeals process
Once you have received your self assessment scoresheet, if you feel you have been scored incorrectly you can submit an appeal.
Instructions for submitting an appeal will be included in the email containing your scoresheet.
You will be given 48 hours to submit your appeal.
You will not be able to upload any additional documents during the appeals process, appeals can only be made using the evidence originally submitted.
An appeals panel will review your appeal. Scores awarded by the appeals panel are final and there is no further recourse to appeal.
2026 Orthodontics ST4 national self assessment form (Draft)
A 'self assessment' topic has been added to the FAQ section on ORIEL to assist you in completing this form.
Listed below are the self assessment (SA) questions and responses that appear on the application form. Your responses to these questions will determine your own score. You will need to support your responses by uploading evidence to the relevant system. The Dental Trainee Recruitment website will give you details of the system and the evidence required. The lead recuiter for the specialty you are applying to, will inform you of the timelines for submitting evidence.
Your evidence will be validated by a panel and used to derive your final validated score.
Evidence of false or misleading reponses will be collated, and you may be referred to a Probity panel.
We reserve the right to use anti-plagiarism software.
Lack of evidence or unclear evidence will affect your score and may lead to your application being rejected entirely. It is the reponsibility of the candidate to provide clear and unambiguous evidence.
Formatting your submission
Please submit a separate PDF file for each of the questions. Each PDF document must be a single document that includes all relevant pieces of evidence. If more than one PDF document is uploaded per question, only the first will be assessed.
You should include all evidence relevant to each question in the PDF. If a piece of evidence supports more than one question, you may need to include it multiple times, addiding it to the pdf for each relevant question.
Please ensure that documents not in English are officially translated or they will not be scored. The translator should sign and date their translation and provide their contact details.
For several of the questions, you will need to provide evidence of LTFT (Less Than Full Time) employment, if relevant to you. This evidenfe can be the same but needs to be uploaded for each question, e.g. timetable of job plan.
The panel will only be able to assess the evidence you provide via thes system. They will not contact you to request additional evidence or access any information you have entered on ORIEL. You must therefore provide all evidence in a clear format so it can be easily identified and assessed.
The recruitment office cannot advise you on your repsonses. You mut select the response you feel you will be able to justify to the panel, using the evidence you provide.
No evidence can be submitted retrospectively.
Evidence should not contain patient identifiable data.
Please do not passwoed protect any of your evidence.
